Head Injury and Concussion - How to Tell If It's Serious and When to Go to the ER!

Most of us have visited the local emergency department for one thing or another. Whether bringing your elderly neighbor to the ER after a fall, or having your child evaluated after a hard hit on the football field, you'll probably visit the local urgent care or emergency department at least twice in your life. Periodically in the news we hear of famous persons dying after a head injury. As a result of the media hype, my ER along all of the others in the United States see an immediate spike in emergency room visits for minor head trauma from fearful patients. That's ok with me, as part of my job is to give you peace of mind as well as to tell you that your not going to die from that little bump on your head.

But when a head injury occurs, when should you go to the ER and what should you expect? How do you know if its really a "serious" head injury? Everyone knows that ERs are expensive and crowded, and sometimes the wait to be seen can be 4-6 hours. No one wants to waste time and money, but if your child falls and bangs his head on the coffee table, and you see a growing purple lump on his head, what else are you going to do? Does he need to go to the ER for a X-rays or Cat Scan? Here are some points to consider when deciding whether you need to rush to the ER or not.

National State Local

Is there a laceration? If there is any wound on the face, you should go. This goes for lacerations as well as periorbital trauma (trauma to the eye sockets) and nasal or oral trauma. Scalp lacerations smaller than 3mm can usually be managed at home if there isn't significant bleeding.

What was the mechanism of injury? The vast majority of head injuries come from falls where the head strikes the ground, table or some other inanimate object. Don't let this variable dictate whether you go to the ER or not. The height of the fall, or speed of object striking a person, is not a dependable indicator of potential underlying injury. When in doubt, go see a doctor.

Is there a concussion or brain injury? This is the million dollar question and the real purpose behind this article. While lacerations and moderate facial trauma may affect some cosmetic and functional outcome, a true brain injury or brain bleed, can cause death or a significant life long disability. So how can you tell if this is occurring? Let's begin with a simple definition of a concussion. A concussion is simply an injury that occurs to the brain from a blow to the head. The brain literally gets rattled around in the skull and can cause bruising of the brain, or worse, axonal shearing which can be thought of as your nerves literally being torn in half. Symptoms of concussion can range from mild dizziness and headache to severe vertigo and vomiting with confusion and inability to walk or remember events. Either way, in most cases is a concussion is a condition in which the patient fully recovers with little or no lasting effects. The tricky part in declaring that someone has a concussion lies in the fact that there is no test for most mild to moderate concussions. No black and white labs or imaging that will tell you a person is concussed. It is strictly a clinical judgement by looking at the symptoms, and if the symptoms are severe enough, you must consider and investigate the possibility of a more head serious injury.

A brain bleed on the other hand is an immediate medical emergency. Blood streams into the cranium putting pressure on the brain. The brain is then literally forced down into the foramen magnum, the hole where your spinal cord exits the skull. The result is that the cerebellum, the area of your brain responsible for spontaneous respiration and cardiac function, literally gets smashed through the foramen magnum, killing the patient. As expected, a patient with a brain bleed first experiences a severe headache and vomiting, then stiffness of the neck and other neurological symptoms similar to stroke, and finally death. If the patient doesn't have any significant symptoms after the first few "golden hours", chances are better that they have a simple concussion and not a bleed.

Do I need a Cat Scan? A lump on the scalp is like a bruise that swells underneath the scalp, but above the skullbones. It doesn't mean there is bleeding inside the cranium. But since bleeding inside the brain case can be so serious, we often obtain a computerized set of x-rays call a CT Scan. The medical standard of care for CT scans in head injuries is basically this: If the patient was knocked unconscious or there is a suspicion of an intracranial bleed or fracture, a CT scan is necessary. So you can see there is some wiggle room for the personal judgment of the medical provider. Keep in mind though, studies show that during a single head CT exam, a person's brain is exposed to the equivalent of up to 300 Xrays! When I order an CT of the brain and skull, I have high suspicion of true injury (or high suspicion of liability I'm sorry to say!). With that in mind, if the patient doesn't have brain bleed symptoms, a CT scan is not warranted as the exposure to high doses of radiation increases the chances of cancer and other complications.

So what's the bottom line? How do you know if you need to go to the ER? Look at the entire situation and make your decision. Basically, if there is nothing to fix like a laceration or broken bone, and the patient denies having a significant headache, and is walking and talking as usual, they probably don't have an intracranial bleed. If you're not sure however, go see the doctor! It's a judgement call on your part and no one will blame you for erring on the side of caution. You can never be too careful and as I said before, my role as a trauma PA is not only to treat patients and prescribe medicines, but also to set your mind at ease that your going to be ok.

Like Katherine Dunham, Lillian Hellman, W.B. Seabrook, Erik Leonard Ekman, Alejo Carpentier, Selden Rodman, Noel Coward and Angeline Jolie, many people say that Haiti is the most beautiful country in the Caribbean. Known as the "Magic Land", Haiti is famous for its culture. Certainly, Haiti its culture, its superstitions and its music. The superstitions or voodoo plays a profound role in the lives of many Haitians. The voodoo was introduced into Haiti in the late 16th Century. Haiti is also famous for its painting, and finally for its ruins...for example the Sans Souci Palace, the most famous ruins in the Caribbean. Exactly, this enigmatic palace is considered a Cultural Heritage for Humanity by UNESCO. Originally constructed by black slaves, now Sans Souci is one of Haiti's main tourist attractions.

Since the late 1940s, Haitian painting, best known as "naïve art" or "intuitive art", is famous all around the World. The most important figure was Hector Hyppolite. His work made its biggest splash in the United States in the 20th Century. Other artists known internationally include Rigaud Benoit, Castera Bazile, Joseph Jean-Giles and Jean-Baptista Bottlex.Haiti is famous for its traditional sculpture. The best Haitian sculptor is Albert Mangoes.

Nelust Wyclef Jean (singer/Haitian-American): Original member of 1990s hip hop group The Fugees. Wyclef Jean is probably the most popular Haitian singer of all time. Jean was born in Croix des Bouquets (Haiti) on October 17, 1972. When he was just ten years old, he moved to the United States. Under leadership of Wyclef Jean and Lauryn Hill, The Fugees had several hits in the 1990s, including The Score (The Score album sold 6 million copies). Since 1997, Wyclef Jean, as soloist, became well-known on the international music scene. Like songwriter and producer, Jean collaborated with superstars as Santana, Withney Houston, Mick Jagger,Bono,Tevin Cambpell, Bounty Killer, Eric Benet, Sarah Connor, Claudette Ortiz, Tarkan, Michael Jackson, Youssou N´Dor, Shakira, Olga Tañon, Carlos Ponce and Julio Voltio. During the last seven years, he has sold more than 10 millions albums worldwide. In 2002, his single Masquerade was a great success.

Since then, Wyclef Jean is a man that always works with love for Haiti, one of the World's poorest countries. Like Miriam Makeba in South Africa or Bianca Jagger in Nicaragua, Jean loves his roots. In an interview for Magazine, Wyclef Jean discussed about Haitian roots: "I am 100% Haitian. I am proud to be Haitian. I still have my Haitian passport. I represent Haiti in everything that I do. Every head in the industry knows that I am Haitian...they know what I'm about. I was Haitian forst. Haitian till die!", said Jean.

Like Gerthie David, Joelle Apollon, Evelyn Miot, and Marjorie Vincent, she has the classic beauty of the Haitian black woman

In 1975, many Haitian people were shocked to open their newspapers and see photographs of Gerthie David Miss Haiti in El Salvador. On the night of July 19th 1975, in San Salvador, Gerthie David, Miss Haiti, was the second black woman to first runner-up in the history of Miss Universe Pageant. After winning the Miss Haiti title, Gerthie went to San Salvador, the capital city of El Salvador, to enter the Miss Universe Contest. The competence was exhausting, very hard, for example Miss Bolivia, Jackeline Gammarra, great favorite, was eliminated. This day, Miss Haiti looks like a black goddess. Her exotic beauty and charming personality are amazing! At 1,72m in height, she was the best in the evening gown competition, but her speech about the Haitian superstitions swayed thousands of applauses in the 25th Edition of the Contest. Certainly, Miss Haiti captive to the judges Sarah Vaughan (American black singer), Maribel Arrieta (Miss El Salvador 1955 and First runner-up at Miss Universe 1955), Jean Claude Killy (French sportsmen) and Leon Uris(American writer ).

When Bob Barker, the host this pageant, announces the final placements, Gerthie was cheered by the entire auditorium. Suddenly, her pulse rate beats at thousand per minute... "First runner-up is Miss Haiti!", said Barker. She was one of the most exotic delegates in all history of Miss Universe. In the 1970s, Gerthie David was a model from Port-au Prince and she became a symbol to the Haitian youth.
Next months, in London, Joelle Apollon, Miss Haiti-World, came in sixth place at Miss World 1975.After sixteen years, Marjorie Vincent, formerly Miss Illinois 1991,won the title of Miss America Pageant. The first Miss America to originate from the Caribbean. In other words, Marjorie Vincent has Haitian roots. Furthermore, she was the second black woman to win the pageant.

SPORTS

Like in Brazil or Italy, the football is the most popular sport in Haiti. Certainly, the national pastime is the football. A different of the Dominican Republic or Puerto Rico, the Haitian people don't like the baseball. In the 20th Century, football became the most popular sport in the Island, thanks to such heroes as Sylvio Cator, Joseph Gaetjens and Emmanuel Sanon. In 1974 Haiti qualified for the World Cup in Germany.

Emmanuel Sanon was one of the Haiti's most popular players and played at the 1974 World Cup. Sanon made a great contribution to Haitian football because he played in more World Cup qualifiers any other Haitian. He is still very popular with local fans. Other phenomenal talent was Joseph Eduard Gaetjens. He represented both Haiti and the United States. He made FIFA World Cup History: When scored United States opening goal in the 1950 World Cup against England. Joseph came to the United States in the 1940s to play in the American Soccer League. His beautiful play in the Brookhattan Club made in a national star. In 1953, he played for Haiti for the first time.

It's practically impossible to talk about Haitian athletes without mentioning Sylvio Cator. He is a legend in this sport. Cator won the silver medal in the long jump at 1928 Amsterdam Olympics. Cator returned home to a hero's welcome. From 1928 to 1930, he was record man in the long jump. Cator was a marvelous jumper and footballer. He also played an important role in popularizing football in the Island (it was the captain of Haitian National Team).

Since then, he is an example for young people in Haiti. He died in November 1938, but today the people of Haiti still think of him with respect. Many years after Cator's death, the National Stadium in Por-au-Prince was renamed in his honor. Cator was the first of the great Caribbean sportsmen that would come to dominate world track and field.

He became the first Haitian to Olympic finalist since Yves Jeudy (boxer) in 1976. He won the silver medal at the 1994 World Junior Championship, the bronze at the 2001 World Chanpionship and the gold medal at the 2002 Central American and Caribbean Games El Salvador. Dorival is one of the best sportsman in the history of Haiti.

Christian Codependency: How to Recognize Burnout

Are you looking for Christian codependency help that will enable you to recognize burnout? "Burnout" occurs when you have pushed yourself too far for too long (usually because you believe you have no other choice and God wants you to), and as a result you find yourself emotionally, physically, spiritually, and/or relationally spent. It is important to know how to recognize burnout so you can make adjustments before there are long-term negative consequences to your ministry, job, business, body, or relationships. Here are the signs:

Resentment toward others for not helping. When you notice an increase in your feelings of resentment toward others and find yourself frequently irritated that they aren't doing more, it is because you are doing too much. You need to do less or ask for more help.

"Doing" out of compulsion. The healthiest motive for doing anything is because you choose to. You can take control of your life by paying attention to the things you feel you have to do and evaluating them to see which are truly necessary. You can then choose to do the things that you want to do either because you like them or know you truly have to.

Loss of passion and enthusiasm. This is a sign of exhaustion because it tells you that something has changed. This is especially true if you loved what you were doing before and now you actually dislike it. The cure for loss of passion and enthusiasm is to rest to recoup so you will be re-energized to experience enjoyment.

Emotional fallout: depression, anxiety, irritability, insomnia, and feeling overwhelmed. Overworking and overdoing will eventually affect your emotional state. Insomnia is directly related to being overly stressed and the loss of sleep will make you even more tired and moody. If you are struggling this much emotionally, something has to change or things will only get worse.

Complaining. When you find yourself frequently complaining aloud or in your head, there is a reason. It could be related to resentment, fatigue, wrong choices, over commitment, bad attitude, or too much stress and pressure. Chronic complaining tells you that you are unhappy and you need to find out why so you can address the issues.

Health problems. Your body can only take so much before it is physically affected by stress and fatigue. Pay attention to the types of health problems you have especially when your doctor indicates a relationship to stress. Pushing too hard is never good for your body. Rest is absolutely necessary.

Feeling disappointed with the outcome of your efforts. It should be no surprise that being exhausted can actually make you perform less well but it is also true that it tends to make you focus on your imperfections more and make your expectations unreasonable. If you are disappointed, spend some time figuring out the cause.

Stress-related family problems. Burnout affects your relationships. You will be more irritable, short-tempered, and difficult to deal with. When you take out your exhaustion on your family, there will be problems at home. If this is happening, you need to make some changes in your life.

There are many things that God expects us to do; but there are many other things we do because we believe we have to when we really don't or we need to learn to do them differently. When you are experiencing burnout, you need to figure out how to take better care of yourself by asking for help, learning to say no, and resting to recoup. Jesus told us that we can learn to take on his yoke to bring us rest in the midst of life's stressors (Matthew 11:28). Recognizing that your Christian codependency has contributed to your burnout is the first step.
